Contact Us Fields

Hi,

I know this has been asked before and I did do a search on it, but I still can't figure out how to add numerous additional fields to the contact us page.
I am using an outside script for my contact us that I implemented by using the ez pages. I really need to have customer address and phone fields in the event a customer would like to request a catalog, party info etc. Has anyone figured out how to do this yet?
Please don't yell if it was posted and I missed it. :cry:
