Products being purchased with no attribute-no charge

We have had an issue this week in our 1.3.7 cart with products being purchased by customers with no attribute listed in the order and not being charged at all for the product. We have been unable to reproduce the problem in our testing and don't know where to look for a clue.

Here's how the products are set up:

• All products are Priced by Attribute with product price 0.00.
• None of the products have "Product Is Free" checked
• All products have the same attributes except that some products also have a download avail.
• All products DO have a default raido button attribute.
• According to what we know so far, each order was intended to be for the default radio attribute.

The orders_attributes table is showing these affected orders having products_options_id=1 and options_values_id=2 which don't exist in their respective tables.

Since I first posted, I found a post (I believe from you) in another thread that said so add an additional read-only option and set it to default. I have done that in the last 48 hours for the products that have multiple values for that option. I guess time will tell if that does the trick.

If you could take a look, though, and see if anything pops out at you, that would be fabulous.
