'Contact Us' - turn off required info?

I've added 2 additional fields to the Contact Us form and removed the little stars, but I'm not sure how to disable the required info parameter.

Also, for some reason, it says 'Home>Contact Us>Contact Us>Contact Us' at the top of the Contact Us page (breadcrumbs). Let me know if you need more details.

I guess nobody can help. I've removed the *required parameters for the appropriate fields, but I'm still not sure why the breadcrumbs would say "Contact Us" three times.

Anybody out there?
