Can't Add Special Instructions during checkout

When I checkout and add "Special Instruction or Order Comments" into the field in page 1 or 2 of the checkout process it will disappear when moving onto Page 3 of the checkout process. If I click Edit on page 3 it sends me back to page 2 where I repeat the process.

Anybody have any ideas here?

Just missed the edit time to add in Zen Cart version 1.3.8a, I am using Paypal Express Checkout and until about 5 days ago this worked, that's the last known order with the comment there lol and I am racking my brain trying to remember what, if any changes I made around that time, only thing I can think of is that I removed the Paypal IPN but can't remember what date, I also install Quantity Discounts around the same time, when I go from Step 1 and 2, it's there, go to Paypal, when I come back it says None in the Special Comments area....
