Need Attribute Help for Prescription Glasses Website

Hello,
I am currently working on a prescription Glasses website.
Our site requirest the buyer to enter their eyes prescription online either through attributes or by uploading the scanned prescription.
Please let me know if it is possible to display the attributes as shown in the attached pic.
Also is there any way i can ask the buyer to upload the scanned prescription while purchasing the product.

You essentialy have 7 attributes here...create them as text in the attribute Option Name area - select text from the drop down...

This will list them vertically and then there is a module in the downloads section to assist in display horizontally

I don't know what mod kobra was referring to, as this didn't exist in 2008, but Flexible Attributes will let you control the arrangement of attributes in many ways.

R(OD) Quantity 2 Boxes ( +$45.90 ) L(OD) Quantity 2 Boxes (+$45.90)

This layout requires that the right and left elements alternate. Organize them in sort order like

R(OD) BC
L(OD) BC
R(OD) Dia.
L(OD) Dia.
R(OD) Power
L(OD) Power
R(OD) Quantity
L(OD) Quantity

Then you can give all the elements float: left;, and the appropriate (R) elements clear: left; so they start new lines.

I installed the mod... but kind of stuck.. can you lead me through this process.
