Attribute required for text

I have selected yes on that flag several times, but when I test it, it still lets me go through without entering data in the text box. Here is the link:

Is there another control that needs to be activated?

What version of Zen Cart and what add-ons are you using?

Clean Install or an Upgrade?

What happens if you switch to the Classic Template vs the one you have?
