Remove Store Name, Address, Phone etc, from Contact Us Page

Hey Guys!

My client wants the Name, Address, Phone, etc parts of the top portion of the Contact Us page removed. Anyone know what file(s) need to be modified in order to do this?

Here's a link to the demo site:

Any help with this would be much appreciated!

Just turn it off in Admin->Configuration->Contact Us - Show Store Name and Address

I do not see "contact us" under Admin>Configuration.

Am I missing a file?
