Selected attribute not showing in the order?

We have 2 orders with the same problem:

Each product comes in two size options. But the order doesn't show what option they selected.
Also the order shows that they've selected gift wrap but we talked to the customer and they don't remember selecting it (we have the gift wrap contribution installed)

Has anyone encountered that problem? Are the two normally compatible?

Do you have defaults set on the Attributes or do you have them set where the customer must know to select an attribute?

It is always a good idea to add an extra Option Value such as:
Pick one
Select from below
What do you want?
etc.

and set as:
Sort Order 0
Default YES
Display Only YES

to force a proper choice by the customer ...

A default is selected automatically, the customer can't click 'Add to basket' without having the size selected here for example.

I'm not sure we have the correct settings. This is what we had when the errors happened, is that where I should change it?
