How do I add a Form for customised products?

Hi, I'm new to Zencart and still trying to negotiate my way around :unsure:

I really need to set up a form for my customers to complete that lets them send their customised requirements for the made to measure dresses we supply.

Amongst other things, we supply custom made to measure dresses and at the moment we need to send customers to our old site to complete our Bespoke Measurement Form with their details.

We would like to be able to do away with the old site and concentrate on the Zen one but we really need to get a similar form up and running on Zen before we can do that.

I'd be grateful if there are Zen masters out there who can assist on this. :smile:

This sort of customisation information would normally be handled by using attributes.

I'm not really wanting to add a form to each listing. I would prefer to have a separate form (this is because some listing contain more than one item for which measurements are required).

I have tried creating a form using attributes on one of my products but it is not showing up on the finished listing and I haven't a clue what I've done wrong!

I would like to support what kuroi says.
We are door hardware spe############t and also sell stainless steel numerals and letters. For all these products we use an attribute of type TEXT which presents the customer with a text box to enter their required numbers and or letters, see example here: http://www.bluewolf.net.au/zencart/index.php?main_page=product_info&cPath=42_182&products_id=438

Now I see no reason this won't work for you eg:
Bust:
Neck:
etc.
simply create one attribute ot type TEXT per measurement you need and all should be cool.
