Required Text Attribute Not Displayed

I've created a textbox attribute and assigned it to a product.

I'm struggling with adding a required textbox for each item added to the cart. The textbox is marked as a required attribute, but the actual textbox does not show up with the product selected. I see the comment field and the title of the attribute, but no textbox for the customer to add the reqired information.

Any help would be GREATLY appreciated.

Check the source code of the page and see how far the code gets. Is this in IE and firefox? Make sure the text option_name doesn't have any values assigned to it. Is this a new installation?

What version of Zen Cart are you running?
What modules have you downloaded into your cart.
