one page order form

I am trying to set up a one-page order / payment form. I don't need user registration, I'm only selling one product, I just want it as simple as possible.

Here is an example of what I'm trying to do: besoplex

I browsed the mods and searched but couldn't find anything even close. Anyone know a way to do this with ZenCart?
