how to add extra infomation need to new customer sign up?

Would you please help how can I add extra customer information need to collect with they sign up? I need to collect the customer's medical license number when they sign up. Please advice.

but it contains a warning which would make me think twice about installing it.

Click on the Downloads link at the top of this page and search for Additional Customers Fields. This is what Stevesh was trying to link to.

Adding extra fields at account sign-up is quite a complex process. It not only involves the user-end but also the db and the admin side. A lot can go wrong.

I think Stevesh is right to question the mod. A lot of people have had issues installing it. Also, as far as I'm aware, it's no longer supported by its author.

If you have a test site and you feel confident with code, then you might want to have a go with it. Otherwise, I would suggest thinking about a workaround.

How bout just rename of of the text filed?

Yes, it would probably be a better idea to rename an existing, redundant field. This way, you might get away with only touching the language files, and possibly the JavaScript as well depending on whether you want validation.
