How can I have form object below attribute name?

How can I have form object below attribute name?

example here:

Name
|__________|

Country
|________|V|

etc..

I tried to add a transparent image and play with "Image Styles" setting but did not get that to work. Well that works only with check boxex and radio buttons I guess?

I know ppl have done what I need but how?
The problem is that having them same row is ugly when using many attributes, cause the form elements start in different left posistion.

I need to have the "Option Type" like text, drop down box etc. to be place below the Option Name . Something like below.

Name

Country
|________|V|

etc..

I am not sure how I can explain it more simpler.
Sorry my mother languge is not English.
