Add a quote form to website
I am working on a site for a freight handler.
Shop owner wants a page that will have a quote request form that the customer can fill out and send to him.
Basic info needed is
Cust name
address
phone
fax
Detail info
Select the zone (A thru D+)
Select the weight (100 lbs - 10,000 lbs)
Date needed
That kind of stuff.
Is this possible?
I've searched and searched and couldn't find anything other than this:

and I'm not sure this will work for me.
Any ideas???
