Transaction approved but no order captured using Moneris module

I have had a few incidents now (5 - 8) where I have received one or more (Approved) sales transaction receipts from my merchant account provider for an order, but I have not received an actual order.

In one case, the customer had a shopping cart with items but changed his mind about purchasing. In the other cases, the customers were trying to make a purchase.

I am not using Paypal and my merchant account provider is Moneris.

Has anyone else had this problem and if so, how did you fix it?
