How to add a Phone number to Contact us

I would add the phone number in the form "contact us"; I have found this contrib that works perfectly http://www.zen-cart.com/index.php?main_page=product_contrib_info&products_id=331
I would know if is possible to render obligatory this field.
I have tried but this contrib doesn't support this option.

All I did was edit my contact_us page under tools > define pages editor.
