Added a new Mod to the Site and now it is not Functioning proberly

I added a mod that I had a guy develop for me which is based on Scripts. here is the link to take alook at: https://www.coolcarpartsonline.com

Now the mod function proberly in store front but In the admin panel when I add a new item (first item) under any categorie the Select Year will show up BUT if I go to add a second item under the same categorie (Or Sub-categorie) the Select year will not Show up. Iam assuming it is a condition in the script. Here is a link to the script that belong to that function in the Admin folder> <?php
