Credit App for Commercial E-commerce site

I'm developing a commercial e-commerce site for a new client. Buying on the site will be limited to commercial accounts.

Anyone know of a z-c plugin that will allow you to create a form for a commercial account/credit app? I can create one, but I'd rather start with something already created.

There isn't one. That's why there was no response.
