How are pictures oganized and what folder to choose?

Only if your base image is also in a sub-Folder....
images/sub1/image
images/medium/sub1/image_MED
etc

Right, kobra. I should have been more clear in my question. Meant to ask if you had to create the folders by hand or ZC made them when you put the small image in a subfolder via adding a new product in the admin. ...Which has raised another question, does ZC see the subfolders in the images folder the drop down when adding an image?

You have to make any custom subfolders yourself, in your hosting file manager.
Any subfolders directly under the main /images/ folder will show in the dropdown. Sub-subfolders may not show.

Gotcha, Glenn. Is there anyway to upload the _MED & _LRG files in the admin as you do with the small image? Is FTP the only way?

It is possible to load them all by the admin, but more tedious than FTP.
Select one of the images, say image_MED.jpg, in the browse field, and select the "medium" directory to load it to. Repeat with other images appropriately, and finish with the real main image (thumbnail), image.jpg, in the main images directory.
The last image selected will be used as the product's base image.

In a previous version of ZenCart–not sure which, it was some years ago, in the admin there was three fields to upload all three image sizes at once. Know why this has this disappeared? It seems it would be an improvement over the current setup.

You would need still more fields to load additional small & large images via admin, and choosing the filenames individually would require a database field for each image file (including an indeterminate quantity of additional images). It would also require a db lookup for each image at runtime. The current system is considerably more economical and enforces organization, at the cost of that total flexibility.
Just my guess.

No, in the end i did not creat a sub folder in the small images directory.
I did away with the patterns sub folder in the MED and LRG images directory.
It was the easiest option for me.
Like i said previously, i should of paid closer attention to the zc tutorial on image preparation! The part about the BASE IMAGE, is the key, If you miss that part, you could have problems.
