My shiipping modules disappeared

I am not sure why it would be loading twice based on what you are displaying ...

I loaded the code and I am not having any problems with it ...

What files do you see in:
/includes/modules/shipping

make sure that they are all code files ...

select Catalog ... click SEARCH ...

Figuring that if all else failed, I could just upload the original files for the cost/weight shipping table and reenter price bands data, I did just that.

Except it's still broke.

In the Admin>Modules>Shipping page, it loads down to the shipping module just above Cost/Weight Table, and then stops. I don't even have the buttons to delete or edit shipping modules.

These are the files that control the cost/weight table shipping module:

1. includes/templates/YOURTEMPLATE/templates/tpl_checkout_shipping.php
2. catalog/includes/modules/shipping/pwtable.php
3. catalog/includes/functions/cost_weight_table.php
4. catalog/includes/languages/english/modules/shipping/pwtable.php

Now, I went back and overwrote includes/templates/YOURTEMPLATE/templates/tpl_checkout_shipping.php
With the backup I made before I installed the price/weight shipping table module.

I then deleted the other three files that make up the module:
catalog/includes/modules/shipping/pwtable.php
catalog/includes/functions/cost_weight_table.php
catalog/includes/languages/english/modules/shipping/pwtable.php

And I still can't get the shipping function to work.

After a restart to clear the cache, shipping is now functioning, sans the price/weight table module.

This module worked for 8 months, and suddenly stopped when I tried to add a shipping zone that included only the lower 48 states. Any ideas what I did wrong?
