You can first enable the referral code feature in admin -> configuration -> customer details -> customers referral status (set to 2). Then a referral code box will appear on the create account page, and you can change that text to whatever you like, for example, "Where did you hear about us?" (this could be the original text - I don't remember what it was since I changed it)
