Well, permissions at the filesystem level, and the general PHP configuration where certain PHP functions can be disallowed by the server administrator.
But if the server has been configured to disallow the use of the PHP file_exists() function, one would expect that you would be seeing actual PHP errors not only on the download-manager screen, but also many other places on the site.

Could you peek again at the permissions on /download ... perhaps I am miss understanding your earlier post ...

Ajeh! You just made my day. That was it! I was rewarded with an instant green light on the filename, and return of the download button in my test customer's account page. Permissions on that folder had been 774...
