Customer can't get past step 1 checkout

@mvanbus43: As for the file, my bad, it's
"tpl_checkout_shipping_default.php" to be exact (without the 1 tho)
Just check which one your template uses (for example add "AAA" in the first one and "BBB" in the second in a div somewhere around the H1 tag). :-)
However I tested it a bit now, and it doesn't seem to be a valid way. Zen Cart adds the price of the shipping to the total, but shows the last possible shipping method later on in the process (for me it was the 'store pickup').
I'll be looking into it a little more tommorow.

Sorry for the double post, but I actually found a way to deal with it for now at least.
If I go to my admin panel and Modules->Order Total, then select Shipping and turn OFF the free shipping there I will be able to pass the checkout. The drawback is that my client will still be able to choose to pay for the delivery, but that may not be such a bad idea after all. ;-)
I can also set the sort order for the freeshipper and free shipping to a high one so it's the first choice (should get selected as default then).

Ok.... crossing my fingers but I think I got it to work.
Apparently, I did not have the ot_shipping module installed under the "Order Total" modules. Enabling that with freeshipping enabled for orders above 0.00 seems to allow customers to proceed to step 2.
I still wonder what the underlying issue is, as this worked just fine without that module installed in 1.3.7

Just to confirm, in testing mode for authorize.net, should I get:
"There has been an error processing your credit card. Please try again."
on the final step of confirming the order, or does that mean something is still not working?

The message means what it says.
Doing a real transaction (not in test mode) will give you more meaningful information. Plus, that way you'll be able to see the transaction in your authnet merchant console and deal with whatever problem it reports there, if any.

Yes.
Why do you have Admin->Configuration->Sessions->Recreate Session set to False?
It should be true unless your hosting server has specific problems with giving you the security protections offered by having it on.
Why do you have so many order-total modules disabled?
