What happens if you make a new Zone Definition and in the DETAILS you add Country for Canada and on the Zone just pick Ontario ... then add that as the Zone on Free Shipping Options freeoptions ...

I don't believe you have things setup right on your Taxes and that you added a new Zone for Cornwall but I think you need to set that up differently ...

Naturally, I am not Canadian and am kinda clueless how your taxes work there, but I do not believe that you setup the taxes just for one city ...

Taxes are mostly provincial. Some provinces have the same rate. Shipping is GST and is national. Pretty confident I have it right.

I set the one up for Cornwall because I still need to charge them tax on the sale. Which what I've been referring to with regards to my zones being province-based.

The store is live. This is my second ZC store over the past 5 years. Never had any complaints / errors regarding taxation.

Do you have zones set up for every province? If so, why? Technically the rule is usually that you charge GST to everyone, but HST/PST only in the provinces where you've registered to collect and remit it (which is usually only the provinces where you have a physical business presence). ie: taxing all of Ontario with HST is fine.
But, taxation is not the subject of this discussion. Leave that for your tax accountant.

Do you have ANY other reason for a Cornwall zone OTHER THAN just this shipping thing? If not, I suggest deleting it. Then let the custom code changes Ajeh mentioned kick in and specifically offer Free Shipping only when the city name entered is Cornwall.

Delete that. Leave it only as a city.

Delete any zone established for Cornwall specifically.

Then test the code she offered.

Well, sort of...Not intended to be used as a province obviously..

So I deleted all the zone, zone def rates..etc. But now what do I select in freeoptions for 'Shipping Zone'?

Okay. so I tried with no restrictions for shipping zone and the both flat and free show whether I'm in Ontario or any other province. So that won't do.

If I add the restriction to just Ontario hoping it will pickup and offer free to just Cornwall it doesn't show to anyone.

Create a Zone that only includes in the DEFINE the zone for Canada and Ontario ... set that to the Free Shipping Options freeoptions and see if that, with the custom code that I gave you, makes it so that shipping module only shows for the city of Cornwall ...

No luck. Still only shows flat rate.

Login as the customer ... then go to the My Account and edit the address it should make you reset the Provence to Ontario ... then check your shipping ...

Yes and on the DETAILS you should set
Country: Canada
Zone: Ontario

Then edit the Free Shipping Options freeoptions and set that Zone on it ...
