First, I want to thank you for sticking with me and trying to help me get this issue resolved.
I am using Paypal express checkout
I used the recommended settings which are
Set Order Status
Processing [2]
Set Unpaid Order Status
Pending [1]
Set Refund Order Status
Pending [1]
And, when the customer completes their order using Express Checkout, what do you see in the actual order as the status assigned to the order?

Actually, even more than that, go to Admin->Customers->Orders
The first column is the order number.
The second column shows the payment and shipping modules used for the order. What's listed there for this order?
A few columns later it lists the order's status. What's the status assigned to that particular order?

I am assuming that the Gift Certificate is being sent to the customer via the Admin ...
Have you tested this yourself with a Customer Account that:
1 started out WITHOUT a Gift Certificate balance
2 started out WITH a Gift Certificate balance
What do you see when you get to the checkout_payment page?
Do you see the Redemption Code for the Gift Certificate in either?
Do you see the Apply Amount: in either case?
If you do not see the Apply Amount, once you redeem the Gift Certificate do you see the Apply Amount?
If you then Apply the GV amount to the order, does it cover the full order?
Once you get the GV amount applied to cover the full amount of the order and can checkout ... what is the Order Status?

DrByte I see 'Processing'
I see enter redemption code but do not see apply amount and after I redeem the gift certificate it takes me to Paypal and wants to charge me.
and if I go through paypal and pay for the item I still end up with the same message
NOTE: Downloads are not available until payment has been confirmed

Okay, so that means the order was never paid for via the PayPal module. It was paid by Gift Voucher/Discount Coupon.
And, the "pending" status will have been assigned by your FreeCharger payment module.
You can change the FreeCharger's order-status by clicking on that module, clicking Edit, and changing the status to what you desire.
"Processing" implies no more payments are required on the order, and thus it's ready for shipping/download/whatever. ONLY USE "Processing" IF THAT'S WHAT YOU REALLY MEAN (otherwise you might end up giving stuff away without getting paid).
Usually I'd recommend setting FreeCharger to "Processing" anyway.

Could the inactve coupons have anything to do with this? I have tried to delete all coupons previously used but it will not allow me to delete them completely. Is there a way to do this?
